Clicks in ISM DTX for CNG (ACELP) -> active (TCX) transitions

In ISM DTX, there can be discontinuities at the transition from a CNG frame (ACELP core) to a TCX-coded active frame, e.g.:

This can be fixed by applying the same transition smoothing as in MDCT-Stereo DTX. Same signal then looks like this:
