The function `read_indices_fx()` is defined but never used, instead `read_indices()` is used. It would be nice to keep only one variant and remove the other one before the porting of CRs starts. Also, there is a switch `IVAS_CODE_BITSTREAM` which would be nice to clean out, and harmonize functions `reset_indices_enc_fx()` and `reset_indices_enc()`.
