[non-BE] MD coding in ISM inactive frames

Fix MD coding in ISM inactive frames:

  • use of low-bitrate mode with MD in inactive frames
  • changes in ISM signaling
  • NULL metadata objects now also use the adaptive bit-rate distribution

Non bit-exact operating points: ISM format for bitrates up to 48 kbps per object + all ISM conditions with NULL metadata
